You should have 6 jumpers in the place of the two missing switches. Move the jumpers for the phase to the left two pads instead of the right two pads.

Repost the DC voltage on pins 1 and 7 of U107

Remove the phase switch jumpers and connect a jumper between pin 7 of U106 to the bottom right jumper on the master slave switch (leave the m/s jumper).
