You can get them anywhere and they are called a diode bridge. Do a search on "diode bridge" on mouser and click rectifier something a couple of times and you'll end up with a couple of thousands to select from.
I can't really recommend any, just make sure you get one with the correct or above rating.

Usually not. Single or dual diodes in to-220 or to-247 packages do however need isolation.
