It's in the article somewhere (http://www.firstwatt.com/downloads/F5-om_sm-080527.pdf) - I think it is should be rated for 30A or 25A and 250V. You can get them anywhere and they are called a diode bridge. Do a search on "diode bridge" on mouser and click rectifier something a couple of times and you'll end up with a couple of thousands to select from.
I can't really recommend any, just make sure you get one with the correct or above rating.

I't probably doesn't matter much. They don't put out as much heat as output transistors.

Usually not. Single or dual diodes in to-220 or to-247 packages do however need isolation.
