Q6/ BD681 is placed too far on the heatsink, which means poor thermal tracking. Would be better to place it somewhere in the middle of the heatsink but you don't have the required space on the PCB. Unfortunately you would have to redesign the whole PCB. The best solution would be mounting it directly on one of the output transistors.
