Yes the NTC 10K need to mount on amplifier's heat skin.

With bootstrap you can get rail to rail peak voltage on speaker... power will be 90W/8R with +/-40V supply. Without bootstrap there will be about 75W/8R.

If the PSU has sagged to 40Vdc then the quiescent will be much higher, probably around 45Vdc.
Starting with a quiescent PSU @ +-40Vdc I would expect around 65W to 70W into 8r0 from a really good amplifier built to deliver lots of current.
