UPDATE!!!

Assembled one channel and powered it up. A few minutes of trimming and that's it. Flawless operation with pure music and no ill effects. These "black-beauties" are ready to fly!!! :)

Using KSE340/350 at VAS, Panasonic M and Nippon Chemi-con SXE electrolytics and everything else pretty much same as before.

This is probably a stupid question but I'll ask it anyway (I've fried LM3886 chips before). In some pics you posted earlier it shows the ground to the heatsink...but where is that connected on the board itself? Also, the holes for bolting the chips to the heatsinks...do we need to use something non-conductive to bolt it down or does this serve as the ground?

When being tested stand-alone or without proper enclosure, the heatsink can be connected to the board's PGND header (which I did), or connect a wire from the heatsink directly to PSU's ground (which I do if previous way is not possible). This is only for test purpose and is a temporary setup.

If you install the boards in a proper metal enclosure with heatsinks then no need to do any of the above, just connect a single wire from PSU ground to a central area of the metal chassis.

Also, the holes for bolting the chips to the heatsinks...do we need to use something non-conductive to bolt it down or does this serve as the ground?

The four plated holes are not Ground and have no electrical connection to any part of the board. So you don't need any insulation between the bolt and the holes.
