Thought number one is right. MOSFET drivers usually use hysteresis. So when it snaps the MOSFET on, the slow signal at its input turns it back off again. It happens many times in rapid succession during what should be one clean output switching period.
About your second thought, the basic topology is simply inherently slow, like you are thinking. Best Regards.
