Has anyone experienced this issue? I'm about to buy a used UA21292, but....when you put on a cable that is unbalanced, eg. pin 3 grounded, the meters go off.
The manual says that this sort of cable is OK.

Differential to single-ended (or unbalanced, as you call it) conversion can not be done by grounding one half of the signal to the ground; well, not if you want to do it properly - the protection circuit kicks in most likely.
