LM4040 from National is pretty nice and really cheap.

Can't remember how the PSRR is, but the noise is very low.
ADR421 from ADI comes to mind as well, but I think that is only in surface mount. I will have to dig way back in my notes, but I can remember using a transistor Vbe in reverse breakdown as a reference as well. We don't need something accurate, just low noise with good PSRR.
