Here (EU standards) we have both possibilities. I showed

the case of double insulated isolation transformer. There is no PE and you may connect only one instrument to its secondary outlet. The standard can be seen in the photo.

No, only the ones I've seen. I'm not certain that an exception could or could not be made for medical rating.

One interesting exception to American code is that a GFCI outlet can be installed in a location where no PE wire exists (two wires, like my old house) requiring a label on the face describing the sin lurking within. Please don't sue me, but this could be interpreted as meaning that PE could be broken if a GFCI were somewhere in the path. Maybe. Bring a lawyer.
