Noob DBT question

Searching around the interwebs, I haven't seen a single DBT that includes an earth conductor. Is there a reason for that?

It can, and should, be used. It just carries through, like the other phase of the AC line.
Slit a spot in the center of the length of a three wire IEC cord sheath open.
Cut and use the hot black conductor for the bulb, leaving the green safety, and white neutral, wires uncut.

That's what I figured. I just found it strange that not a single example included one.
