I decided to try one more time with toroid.com, having only essential windigs this time:

Primaries:

Secondaries (total 5 windings) 
bifilar 2x 38V RMS at 12A RMS (39.5V 0a) 
bifilar 2x 60v RMS at 0.8a 
single 18v x 0.8a 
  
STATIC SHIELD between primaries and secondaries.

Prices:

2-4 pcs: $362.52
5-9 pcs: $250.93
10 and more: $209.93

So there may be a chance of them at $250.93 🙂

Let's start with $250 price level, which is lower then 10+ from previous quote and close to prices from off the shelf transformers without shields/screens and non-bifilar secondaries

Good, looks like $210 price level is possible 🙂
Please ask bobgroger about the boards in the main thread. Worst case scenario you can take gerbers from him and submit them to your favorite PCB manufacturer.
