Ok so there is still an issue. It plays audio at low levels. As soon as I turn it up it starts distorting when the bass hits.

Have you tried using a test tone? 50hz would do.
Do you hear the relay click on and off other then power up or power down?
You said you made a mistake when fixing it. What did you fix and what was the mistake? This might help understanding the current issue.
Yes st low levels its clean.
I was using a test tone 66hz I believe. Ive tried different frequencies.
Relay clicks on at startup. Thats it.
When i was reflowing the Bus bar for the ground I accidentally made a connection to surrounding part of exposed board.

My power supply is current limiting so I dont think any damage was done. As soon as i removed the excess solder it powered up fine. Plus I had the issue before I did that.
Are u sure u didn't mean use the audio ground? Off of the main ground I only get mv's for everything. if I use the speaker neg. All the measurements seem right on

Also, look at the LM361 with a scope. Pin 4 should have a triangle wave. There are two test points at the corners of the class d board. Post your DC measurements and screen shots.
I unclear what's going on here and I need the info to get a better picture
