May I suggest you consider a different means of collecting information? That is quite a lot of personal information to disclose to just anyone on this forum or the web who clicks the hyperlink.

Nikon1975 your spreadsheet is non editable. It asks me to send you an email so you can grant me access to the sheet. This makes no sense. I suggest you ask people to PM you with the required information.

So I collected the info for people who sent messages. The Excel file should open for modifications for who wants to use it.

I'll check this morning for shipment costs and sent request for payment today. I'll ship the paid items as soon as payments arrive.
