Voltage doubler caps C15/C16 voltage rating seems insufficient at 63V. I estimate you have as much as 68V across each one at nominal line, 100V caps would provide some margin.

Two el.capacitors in series (if one is 63V) is then obtained 126V!😉
It's OK C15 and C16 to be 100V!

And across those capacitors under a light load you could have as much as 136V (depending on transformer winding dcr it could be a bit more or less) at a nominal line voltage of 230V.

Have you actually measured the voltage across those caps? 😀

Voltage across those caps is 132,8V (Voltage across one caps 66,4V!)😉
Sekundar DC-Voltage Us=48x1,41 = 67,68V
