Have you forgotten your amplifier enable line? Pin 5 of J3 on the SMPS to ON/OFF control pin of amplifier.

Im in the mids of wiring up my UCD OEM as well.
Am going to used single ended input. May I ask
how is this connect up ? Also at default I believe
the module is in stand-by mode. How do you trigger
the module out of stand-by mode.

Yes disconnect that. The pins you have connected to on J5 are Aux power supply outputs.

Connect negative input and ground together at the chassis ground point on the rca connector and you are good to go. You need to connect on/off pin to power ground to trigger module on, as per the manual.
