Nice work, output on A17 must be on botom (cooper) side of pcb (pins are reverted), NJM2068 instead LM4562 can be use.

I thought so(the marking doesn't look good), but maybe it is original one...The jengkols do not look like original, John. But it's not way better than 3055 anyway
I saw at post #3317 there are AX-14 also use jengkols (with better marking)
but don't worry I have spares some old motorola 15003-15004 if this one is blown
I will try at rail +/-55VDC for 8 ohm load, is it is okay?
if they were original I believe they wont fail
