left channel: pins 1,2,3
Right channel: pins 5,6,7
And so it would appear the left signal leaked into the right signal.?
I would not go as far as to say the signal is "clean" since there appears to be noise everywhere in this amp but the signal is present and clear on these pins.

I don't understand what they're doing with U2. It's strange to have the same signal on both the inverting and non-inverting inputs (with the op-amp out of the circuit).
Hopefully, when you install the new op-amp, it will work properly.

So what do you think about the trace that seems to be missing? I connected it and the amp drew enough current to pop my 10amp fuse. It also heated up two traces on the top of the board. I have attached images of the overheated trace. As a note after removing the connection of the trace the amp functions as stated in my most recent post, so it seems no damage was done...
Also I have attached an image of my amplified output signal. It seems so noisy...but I am very new to all of this.😱

If the trace was cut, it could have been a layout error. It happens fairly often.
At this point, I'd suggest replacing the op-amp to see if the amp will work properly. Clamp all transistors tightly to the heatsink before applying power.

I pulled the board out of my 604x late last nite. When I get home this evening I will flip it over and report any irregularities I might find with the traces in the general areas you pointed out in the attached photos.
Sorry it took a minute to get to this thing but I will have an answer for you tonight.

@ Cabron, No problem, I appreciate your looking.
@ Perry, I have ordered some 2068s and will report back as soon as Mouser gets them to me.

Well I'll be damned. I never would have noticed it if not for your graphic but it
is defintely there.
This is just my opinion but to me it looks like, for whatever reason, that it is
there intentionally.
I can't come up with one good reason for a trace layout like that but I am by no means an expert.
Just so you are sure we are talking about the same thing, the area in question is almost directly opposite of D804 & D805 or somewhere very near there.

Cabron, Thank you for looking and yes that is the exact area I am talking about.
I think Perry is right about it being a layout error.
I did receive the 2068 today and after installing it the amp functions as it once did!
I will burn in the amp for a reasonable amount of time but I feel this case is closed. Thank you Cabron and Perry Babin for your help. This was certainly an interesting problem and I am very pleased to use this amp again!😀
