I do not use preamps (except for a phono preamp).
I have a passive remote stepped attenuator.
For other systems, I use potentiometers.
All my single ended amplifiers were and are 2 stage, just as I said.
However, if I was going to use a single ended amplifier in the living room to work with the TV, I would need either a 3 stage amp, or a line preamp.
My TV has Very Low output L and R signals.
In that case, a 6SN7 would be ideal for a 3 stage single ended amplifier.
Not too much gain in that case.
All Hi Fi and Stereo are Systems (signal source(s), preamp(s) if any, amplifier, loudspeakers, room, etc.)

Yeah, I expect about 300mV or so from a TV HP output, but through a 1:5 step-up that becomes more useful.
Perhaps I will check the Unrated output impedance of the TV audio someday.
Then, I might find a pair of suitable transformers.
Until then, I have already solved the problem with another product.
I do listen to my TV and DVDs with lots of Stereo sound.
And DVD players have audio outputs, but that audio output does not sync with the HDMI picture.
Isn't digital wonderful?

Have not heard from DoktaDave since! May be he is little frustrated seeing us busy with our arguments.
I'm still here! Audiowize has given me the answer to my original question that I was looking for though, and I don't have a lot to add to these other discussions sorry.

Concerning toroidal transformers with shielding, is it possible to install output transformers outside the chassis and the power transformers inside the chassis? Will the latter cause noise issues? I would like to make the layout compact installing output transformers outside and power transformer inside.
