Maybe this one fits:I looked at these and the 261 i might be able to make fit. but i would have to add a couple of resistors to drop the voltage down a little bit. or the 262B6 but then i would have to add diodes and caps for a voltage doubler. both MIGHT work.

Hammond 262B6 has twice the capacity you need, B+ will be a bit high with a doubler, but filter can drop it - dubler-RCRC and you won't need a choke. I doubt you'll find anything smaller. $28 at Mouser - more than I'd like to pay, but I have several similar parts in the basement.

Lots of people build single-12AX7 stompboxes for guitar. Nixie power supplies are popular for this (DC-DC boost converter). You can buy ready-made PCBs, or build one yourself with a 555 timer and a few parts.
