That is a very well thought out circuit. That chip would have been better for a couple of circuits I designed and built. (I still hate SMD though.)
Is the chip fully protected? I worked with CMOS op amps in the 70s (CA3140) and they were ESD sensitive. CMOS op amps were originally developed for sample/hold circuits but offer big advantages for some linear circuits. In fact I built a preamp using CA3140 chips in the 70s and it was well received.
