You likely need to ground the source leg of the output FETs. It's best done close to the FETs but connecting a load across the speaker terminals will be better than nothing.
I connected a load across speaker terminals and same result .
The outputs are out of the amp right now .

Without the power supply (red and black wires) connected, you can ground your scope probe to the transistor source. You can also use a jumper from the source to ground (use the ground jumper near the outputs).
Without photos of the waveforms, it's VERY difficult to know what you have.

I confirmed I had good drive signal on all of the outputs and replaced U17 and the amp works like it should now .
