This is one instance where a 120v incandescent lamp may help. Connect it on place of the resistor and then, if the problem is intermittent, you may see the lamp blink when the problem makes/breaks the fault.
I had a shorted optocoupler.

Check the output inductors if u got it in portect these amps are bad for shorting inductors. Loosely wound
Pin 16 is too high. Either the IC is defective or something is driving voltage back into the IC.
Five band resistors with a fourth band of gold or silver form an exception, and are used on specialized and older resistors.
