Just for fun, I guess that they are only missing at the end of the first sentence just a comma which would behave like an "or".
If confirmed than it would be: 2 tablets orally or 1 topically (in the ear).
1 tablet in the ear may indicate a veterinary prescription for a dog, just as an example.
No idea why just the left ear is pointed to, but that could have a trivial reason too.
As a COPD sufferer I fear the day when the Scottish Health Service - in its drive toward 'low price is king' - start to use such suppliers!

Oooooooh (not Oak, but Ivy)... I'm sure some people didn't remember the old adage, leaves of three, let it be; berries of white, take flight... and were out of TP on a camping trip. The horrors... makes me cringe thinking about it.
