You could do a lot worse than the circuit National Semiconductor has in their app note for their new LME opamps.

Check out the Hagerman Technology Bugle preamp kit; those are available on eBay for reasonable prices. Reviews suggest that they perform very well.

There's enough info there to build one from scratch if you wanted.
If you listen to house and techno music, you could build a classic DJ mixer phono stage: the schematic for the UREI 1620 can be found online. There's some images online of the 1622 phono card which clearly show the layout.

I strongly recommend against "passive eq" with flat op amp gain stages in RIAA preamps as shown in the national datasheet:
