I can make prototype PCB's - the PCB would be 1oz copper on a FR4 substrate, 2-layers, solder-mask, and through hole platting. Here is an example of a board I made prior to solder-masking. I can also provide PCB design services.
An externally hosted image should be here but it was not working when we last tested it.
I wouldn't necessarly call these DIY PCB's - although if you have deep pockets.
They are milled on a LPKF C100/HF. There are a couple of methods for DIY through hole plating. The easiest is rivets:

Another route is using conductive ink - lpkf sells a "consumable kit" that has everything - here is a video:

In general - LPKF is super over priced. You can get the conductive ink from thinktink.com and he has a good discription of how it is done. LPKF stops at application of through hole paste / conductive ink. This is ok for "short term" PCB's - but if you want it to last you need to electroplate it which actually is not as involved as you would think. Thinktink has great prices and there stuff is super high quality. We got our vacuum table from clampusystems:
Vacuum Tables, Vacuum Clamps, Vacuum Pods and T-tracks from Clampusystems.com
They fabricated a custom built table that is perfect - hooks up to a shop vac. If you do get a table from them, tell them Max from the University of Minnesota referred you. Ours cost $600. If you are on a super tight budget, you can used just a standard shop vac with a sock over the end to suck the paste through.
