For some reason I wrapped the servo loop around just the output stage rather than the output and opamp stage.
It was a while ago now, and not sure why I went that way. I have a feeling it was that the sim wasn't happy when I tried.
It does work pretty well this way anyway, so maybe I got lucky.

Try measuring the voltage on the servo opamp output. Is it close to a rail?
You aren't giving much room for the convection cooling of the Burson opamps, they run quite hot you know... Perhaps remove the plastic cases from them?
I made sure to put ventilation slots on the rear face of the enclosure so that there would be airflow between each level out to the vented top.

On my to-do list I'll add putting a thermocouple on them to see how they actually perform with everything buttoned up as it'd be interesting to find out.
