Seems like the compensation of the circuit is killing THD.
Was a assumption.
Bandwidth would likely hold up better with 4562 at higher voltages.
But the compensation would likely get same results.
Dont matter, cant thermal track with Surface mount, and second drop still not compensated.
R1 C10 C4 C10 C5 C6 most that stuff just needs to go.
Thermal track with TO-126 R11 and R15 either thermal track diodes or transistor as diode on heatsink
all transistor goes on a heatsink , expect op amp and current sources of course
