Is it me, or is this ic really fragile? On second pcb I tried, all aux voltages were zero and both clip and fault leds were on, and that was after I replaced the IC. After replacing it again, I got it to work finally.

I have second board working, some input caps were leaking DC. The first board that this thread was about, has channels A and C working, while B and D inputs have DC voltage close to 0 which is probably why CLIP led is on. A and C inputs have DC voltage at 3.8V. I am tempted again to replace the IC, unless there could be something else that could be pulling down these input voltages.
