Need help to wire Buffalo III 9018 with Lundahl 1539

I am adding LL1539 output transformer to old TPA Buffallo III for SE output
Currently I wire like this:
Primary:
Wire pins 2,4,5 to +OUT of IVY
Wire pins 1,3,6 to -OUT of IVY
IVY GND not connected

Secondary:
Join pin 7 to 11
Wire pin 8 to +RCA
Wire pin 12 to -RCA

Sound is nice but hum. Do I need to connect GND to anywhere to get rid of hum?

Hum is usually caused by how your ground the primary and secondary windings, and or the presence of external magnetic fields near the transformers (say, maybe from putting the output transformers too close to a power transformer).

In particular for your case, here is a schematic of the transformer:

Normally what you would probably do is connect, say:
Pin-2 to IVY+ output
Pin-3 to ground
Pin-1 to IVY- output
Pin-4 to ground

Alternately:
you could try tying pins 3 and 4 together, but not to ground, so that the primary winding would be floating.
