I disagee. The input stage is just the subtractor. Even with a perfect subtraction the amp's output will not be a perfect, enlarged copy of the input. Getting the subtractor to work accurately is important but not very difficult compared to the rest of the circuit.

Of course not. In this case I suggested pralleling to improve noise in the first stage. I thought it was implicit.

This is a design that used a CMOS inverter as input and driver stages. I did this approximately 15 years ago, while working on a project that required a lot of CMOS logic ICs.
The design is sound in that an inverter is essentially an inverted open-loop op amp. While the sound isn't top notch, it did work.
I was looking for the same thing- just a basic A/B p-p mosfet amp. Couldn't find anything, so I decided to start working on my own. I don't know if you'd call it minimalist, but it certainly isn't that complicated!
