I don't have Self's book to be able to read page 101 but that number rang a bell over some magazine articles I kept that have become available online.

C3 is just a ripple filter referenced to the filtered positive rail. It cleans up the bias for the two current sources.
LOL, it is not any kind of boot strap.
Maybe I'm somewhat dumb, but I don't get what C3 does?

If you look at any of Self's circuits show, me any with positive supply RC filtering of the small signal stages.
Electricity involves the flow of electrons in the opposite direction to the conventional notion of current. The a.c. reference point is the junction of R13 and R17 in between earth the origin and the positive rail the end point.
