Does any one know where I can purchase a circuit board for the LM4562 Phono Preamp using their schematic?

This would be my first preamp project

What voltage resistors and capacitors would I need?

Resistors could be 1/8 to 1/2 watt, capacitors 50 volt. You should add an output capacitor.
