No it did not, yet I have extra of those and some other that have higher ratings. I still have the transistor that came out of there. I'll can get that exact transistor or a similar transistor in place. I'll change it out before it causes issues later. Thanks
For the regulators (which run too hot), you may want to add heatsinks. The following page has an example:

Those are likely too heavy unless you find a way to secure them. A TO-220 transistor will have more of an integral heatsink than the smaller transistor. If you use them, remember that the pin configuration is reversed.
There is some static in amp1&2 audible noise. I didn't notice it till I just audio tested it. I think I read about it in some post somewhere..
I'll check now. I do know I replaced a leaking capacitor and a op amp that was getting hotter than the rest. It's better. But I must have the RCA feeding both front and rear.
Man those voltage regulator do get hot they should be on the underside and attached to the sink. Via pads.
This may be one of the amps where it appears that they didn't expect anyone to use the rear channels alone so if you only feed those 2 channels, it's noisy. If you feed to front 2 channels or feed all channels (y-cables), all channels should be OK.
