It appears to be just a standard relay. DK or Mouser will likely have several that will work. The only options are really sealed or not and with a cover or not.

I was trying to look it up by part number on mouser and got no return on the search. Google gives plenty of results but they are EBay or other companies I’m not familiar with.

Ok. I figured it out on mousers website. I just had to make sure I was getting form A (Normally open) to get the correct pin configuration.

So one pin on the relay got really hot and damaged the via on the top side of the board. I’m gonna post up a pic of the old relay and the damaged via. I’d be interested in your input on the best way to get a solid connection again.
This board evidently suffered from a poor solder connection and burned itself out over time.
The primary connection to the circuit is on the bottom of course. Should I do anything to the top side?

German Bungard has some wonderful rivets exactly for such pcb repairs.
There are also many sizes.
With a little extra work you can fit them without the special press tool.

Desolder the 4 holes around the through-hole. Weave a wire like the leg of a 1/4w resistor through them, ending with a wrap around the center hole and solder the relay in, wrapping the wire tightly around the relay terminal.
