The supply voltage is too low. Confirm that you have 12v+ across the B+ and ground terminals.

I confirmed i have 13 volts on bot battery and remoe terminals..

Still only 5 volts on Pins 13 and 15

Do you have oscillation on the tab of the transistor you replaced?

The transformer that that transistor drives produces the ±15v supply voltages. Try to see if there is an obvious problem in that area.

I dont see any problems in that area..

I have 13.90 volts on the center tap of the FQP30N06L

Do you read 0 ohms (actually 0.1 ohms) between leg 3 of the transistor and ground (no power applied)?

I get 0.3 ohms

Do the waveforms on your transformer look like the ones in the attached image?
