Is there any burr or rough spot on the heatsink that's possibly cutting through the kapton and shorting to the transistor?
Is it possible that the transistor tabs are touching the clamps when they're reinstalled?
I always check for a short between drain and the heatsink.
The drain is connected to the tab.
Got caught out with swarf before under the transistors.
The 10 ohm resistor near the terminal connector on the driver board sometimes opens and causes the LEDs not to illuminate.

After changing the output section the BIAS is to high.
