OK... this is my last post. I finally got the replacement integrated amplifier in the mail yesterday! I soldered it on the PCB, put everything back together, then tried it out. Let me just say it was glorious. I tried a 30hz test tone with my subwoofer set to 1/5 power and it literally shook my entire apartment. The thing is rated down to 25Hz and I am very impressed with its performance. I set the crossover to about 50Hz, since my Polk Monitor 5A's apparently go down to around 40 or 50 (not sure which). Not only is this subwoofer powerful, it's extremely detailed. I'm hearing details in the sub-100Hz range in music that I've never heard before. Incredible!
I bought the sub broken for $60 and after parts and misc costs, I ended up spending about $130 total. Definitely worth it, if only for the learning experience.
One last note: the volume (power) knob on the amplifier is a little fussy. I might need to replace it soon, but everything seems to be working great.
