Mains undervoltage or short mains dropouts, THE ususal cause of death for (guitar) amps (on stage)... when they use fixed bias derived from an unregulated secondary voltage. Mains drops... more bias on the tube... hard duty (heavy metal, hell yeah)... secondary voltages drop even more, tranny might saturate... a vicious circle. Mains or HT fuses typically don't blow since those are rated to withstand the power-on surges...

In the end it's just bad engineering practice.... this kind of death can easily be avoided.

Sometimes I've seen an amp going down this way, from POV of the audience and had no chance to help... that' s real bummer!
