It's difficult to say what is wrong, I have also built this amp with problems but this was my own mistakes.
So, you can not bias the positive rail so how about the negative rail?
Im my amp I did some extra, I lowered the gate resistor to 270 ohm and putted source resistors in the O/P stage on 0,22 ohm.
Check the connection of Q9 base to collector (I missed this one at first with strange effects in the amplifier.
For me I could not bias the amp to 200mA with the 200ohm trimmer so I changed to 470ohm instead
To search for more errors you can send me the drawing of the pcb or show it here if you want to

It works!!! Aftrer over an year of constructing...
D1 and D2 were broken...now i've replaced every semiconductor on the boards both modules work fine!
