If this is the actual text written ON the certificate, i would think it would look something like this :

Qualification Certificate granted by the Professional Technology Committee


After passing the examination by the Committee on Intermediate Professional Technology, Yu Wei Hua is now certified as a Physician and granted the title of Physician-In-Charge .

Date of certification: December 1998
Certification number: 210064

notice the verb tense change: IS NOW vs. HAS BEEN. I think this is more appropriate for what would be written on the actual certificate.

It sounds like the literal translation would be headship but in English you'd never use that word under this circumstance. Sounds like it's saying "the professional technology that's the head of it's feild" or something like that.
