Help identify IC - YEUM1007A output driver card

Looking to identify the center IC on this YEUM1007A output driver card. Card is from a Sundown SAX-125.4 but the same card is common on many of the class D 4-ch zenon board amplifiers.

Does the pin configuration match the LM319?

It does not seem to coincide with a LM319

Pin 4 is +15v supply. Pin 11 is -15v supply if that helps.

could be a standard quad opamp eg TL084 or opa4134

Sorry to revive this old thread but I have the same problem and can't seem to ID any of the ic's on this board. Tried the IRS2092 on the sides but shorted very quick.
