Try this: take photo, and de-solder the head from the cable, use a thin file / 600 or so grit paer, clean off the pins, check again.
Like I said, corrosion, and also broken cable can cause this.

All working now, but with replacement head bought off eBay. I set the azimuth by ear, which I'm sure is not perfect but it's good enough for me and my 40 year old tapes.

Its perfect if it matches your existing tapes and doing it by ear guarantees that.I set the azimuth by ear, which I'm sure is not perfect but it's good enough for me and my 40 year old tapes.
Its no good setting against a perfect reference tape if all your existing tapes (with original head) were off a little anyway.
