ive never ever seen one fail open ckt? I suppose if no fuse is used it will melt down eventually into an open, but youll be to busy putting out the flames to notice.In fact, I suspect that for most purposes people would prefer a rectifier to fail open circuit so that is probably what they often do.
folks always use a fuse on the hot lead of any DIY mains connected gear.

It's not whether it's a requirement or not. It's about whether the diode will survive longer than the fuse.Do you have this requirement in UK?
If the DN fails to open while the Fault current is passing, then the Live Fault remains on the equipment until the fuse opens.
How will the fuse open if the route for the Fault Current has been broken?
That is until someone touches the now faulty equipment and their hand/body "earths" the live part and closes the current exit route. The fuse now blows just as your body gets flung across the room!
