Sometimes, yes. One example is the cap at a regulator output- a little ESR is often needed to keep things stable. The first cap in a capacitor input filter? Low ESR means high ripple current.

Yes, and you either use a high ESR part or add a resistor in parallel with the cap 🙂
Umm I think you meant resistor in series with the cap 😉 (and something small probably less than 1 ohm).

Considering you mean to say connect series resistor, I would say why not use cheaper Low ESR resistor instead of series combination of resistor and cap. One more thing I would like to add is use parallel Ceramic cap with electrolytic cap to improve transient response of the regulator.
Also would like to suggest that the typical sign of high ESR in filter cap of smps powersupply is the heating of cap, despite of using right voltage. in most case I would use multiple caps in parallel to form the required capacitance. this also reduces effective ESR.

Why? Because resistor wont add any damage and you may have already a stock of good caps, some are not expensive at all (i.e. Panasonic FM).
Before paralleling/bypassing caps, you should consider the risk of resonance summing (see other threads where this phenomenon has been simulated -it's very real).
