I'm working on a vintage Echochord (live music echo effect) analog machine using a 1/4 inch recording tape in a loop. The AC input socket is design with only two wires (no ground). Since the original 120V cable is long gone (lost) I want to replace the input socket with modern two wires and a ground connect to the metal chassis.
With of the yellow or green wire should become the NEUTRAL and the HOT ?

Does not matter. (Plugs in those days could be inserted either way.)
What matters is that your new 3rd wire (also green; do not get confused) MUST go to chassis SOLIDLY. Preferably in a way that it never needs to be disconnected for other repairs.
