Hard to say if something is broken or if its a design problem. To figure that out you would probably need a schematic and perhaps some measurement equipment. If a relay works at all, a bad relay or bad transistor seems not so likely. If you find that a particular relay never changes state, that could suggest a bad relay or driver transistor.
