I can only agree with bucks bunny that the information you give doesn't leave us a chance to tell you what is wrong.
What we need is the situation and the problem.
Actually, the problem you seem to describe is that you have a maximum output voltage which is much below what you expect. That's sufficient for a start.
The situation is only described by a supply voltage of 94V-0-94V that indicates a high power design. You also tell us that it is a class D amplifier.
Attempting to make a high power design work, we can assume that you have at least basic knowledge on how to analyze the functioning of a class D amplifier circuit.
Please provide us with all the characteristics you can imagine may be useful to us when we analyze the problem with the amplifier.

Do you know the gain of the amp, and the input signal amplitude? That would indicate if the problem is lack of gain or clipping/limiting.
