BTW. i added bass boost circuit within the feedback loop and after R4. 0.22uF + 4.7k res
Distortion still remain...
Until i double up my supply to 18v.... I just noticed that 4562 minimum voltage = 5V!!!!
For the NJM4562 the minimum is +/-4 with split supplies, which would be 8V with a single supply, from the data sheet here:

I'm out of ideas at this point on the distortion! Maybe somebody else on the forum here will have some ideas.

i just tried with Gain 2. R2 and R3 = 1k ohm
With bass boost circuit = Distortion at 75% volume from my iPhone
DC Offset burst up to 50+- mV once it passed the "safe" amount of input volume. HD650 and D5000 both give me same result.
Without bass boost circuit = Works!!(no bass)
