Balanced DIY op amp schematic?

I downloaded Nelson Pass' DIY Op Amps .PDF file, and am very interested in trying this out. (I'm a mechanical engineer, so this stuff is a bit of a stretch for me.) He makes mention in the text of a figure 8b, a balanced DIY op amp. However, figure 8b doesn't show up anywhere. I assume it was published in the original article. Does anyone have the original publication, and if so would you be willing to scan and e-mail me the missing figure? Thanks!

Paul, I think the article is referring to Figure 8 and not "8b," which seems to be a typo. The opamp in Figure 8 is set up for balanced inputs. As explained in the text, you can convert it to single-ended input by grounding either the + or - input.
